Postpartum Doulas

Please fill out a shift report for each shift - this is service documentation in case there is an issue over shifts worked and anything of concern should be written in these notes, along with the basics of the shift.

Postpartum doulas should invoice each pay cycle, one form per family, for the hours worked. As payments are done the 1st and 15th of each month, please submit the invoice by the day prior to ensure you are paid on time.

Birth Doulas

Submit after a birth has been attended. I will pay you within 3 business days of receiving the birth invoice.
